Please, ONLY complete this Volunteer Application if you plan to work with students or assist a classroom teacher or staff member while students are present throughout the school year. If you are attending a PTO meeting, working with other parents popping popcorn (as an example), or in another capacity where you are not directly working with students, you are just considered a "Visitor" and do NOT need to complete this application.

By completing this Volunteer Application, you are giving FAPS and Visitor-Aware permission to conduct a National Criminal Records Background Check.Â A Volunteer Criminal History Check will be completed annually if you continue to Volunteer at our school.
