  • MSD of Mount Vernon Volunteer Application

     

    Thank you for your interest in volunteering with the MSD of Mt. Vernon!

    If you're applying to be a volunteer coach for a grade 9-12 IHSAA-recognized sport, you must first contact the athletic director. These coaches are subject to additional requirements from the IDOE and IHSAA not covered through this application process.

    When completing the volunteer application, you must use your full legal name and answer all required questions to ensure proper submission. You will be able to select from a variety of activity types and locations - please choose all that interest you.

    For application updates and volunteer communications, a valid email address is required. You will be notified of your application status within 1-2 business days following submission.

    All information collected in the application will remain confidential and will not be shared outside of the volunteer program.

    By click SUBMIT, each volunteer acknowledges and agrees to:

    1. Abide by all Board policies and District guidelines while on duty;

    2. Be covered under the District's liability policy but acknowledges the District shall not provide any type of health insurance to cover illness or accident incurred while serving as a volunteer;

    3. Not be eligible for workers' compensation, release the District of any obligation should the volunteer become ill or receive any injury;

    4. Be required to report any arrests, the filing of criminal charges, or convictions for a crime of which they volunteer is the subject; and

    5. Be required to report any substantiated report of child abuse or neglect of which the volunteer is the subject.
